Wrong info and patient’s detail written on the thalassemia blood request form especially pertaining patient’s antibody and type of phenotype
blood.
WHAT WAS CHANGED, IMPROVED OR IMPLEMENTED?
A google form (excel sheet) was implemented with pre-recorded patient’s detail and information.

WHAT WAS THE BENEFITS?
1. Time saving as primary team doesn’t need to manually fill in the form (patient’s detail auto fill in once patient’s name selected).
2. Reduce chances of mistake.
3. Blood bank can request the blood stock right away without having to recheck the patient’s blood type.
