REGISTRATION PROCEDURE
Registration of participants is the cornerstone of our existence. It gives us a great deal of valuable information. Most importantly, it shows that we have emergency information and documentation of service.
NO ONE is allowed to participate in any Clinton Township Recreation program until a completed registration form has been received and processed at the CTPR office, or online registration has been completed.
Points of Importance
Please review the parent(s), guardian(s) and or the names of any adult who can pick the child up and/or release to latch key or release to self.
If you get a card/roster with a request for accommodation, alert a supervisor and get their approval before granting the accommodation.
Late Registration
Since participants must register at the office, a family could register after your roster has been run.
If a child appears at your site and is not on your roster, you must check for the following items before allowing the child to participate:
The parent must show a copy of their registration form (to be left onsite with all emergency information – until a new roster can arrive) for that child and your program. Check your roster online to see if they have registered.
If you have any doubt, call the office to verify.
Remember... NO ONE is allowed to participate in any Clinton Township Recreation program until a completed
registration form has been received and processed at the CTPR office and all contact information has been provided to you.
ROSTERS
Rosters are given to the staff for each program. They have a great deal of information that staff should review.
Check the age of each participant. Some youngsters are enrolled before they turn the appropriate age and are told
to attend after their birthday. Check your roster to be sure all of your children are the appropriate age.
All participants must have completed kindergarten.
Participants who turn 15 during the program may continue to participate.
Check for special needs. Children with allergies, asthma, epilepsy, etc. have their special needs listed. They are listed
so that you can assist the child – read your rosters carefully.
Emergency numbers should be listed, use when necessary.
If a child is not on your roster you cannot accept them into your program. Give the office a courtesy call before
turning away any participant – it could be a clerical error. If you find a child is not registered, be sure that you release
the child to an adult – do not send them away on their own.
If a person who is not on your roster comes to you and says they “just registered at the office” you can only accept
that person if they have one of our receipts with a copy of their registration information that has been stamped by
the office. Please call the office before turning them away.
If you are a CDBG site that does not receive an initial roster you must work diligently with your participants to see
that ALL the information is completed on the registration card. These cards will become your roster until one is provided.
NO PHOTO - Please check your member comments for the words “NO Photo”. You will not be allowed to photograph any person that has this noted in the “Member Comment”.
As part of the Township Identity Theft Prevention Program we will be shredding old rosters since they contain personally identifying information of our participants.
Once you are done with a roster you may:
1. Bring it to Steiner at the Monday morning staff meetings.
2. During site clean-up, place them all in the back pack.
