            Dear MPW-IND team,

            Please be informed, our site on 5  September reported the first major injury of MPW-IND, which we targeted not to
            happen in our organisation. An LTI injury Falling down from the ground level and broken his leg bone. Incident
            caused by not providing proper walkway inside the site, not recognising the risk and poor housekeeping.


            We should have prevented the incidence by, “Be caring with your colleagues by calling out their attention against
            possible hazards, and to act cautiously with a “maybe/doubtful” mindset”.


            Recognising the risk is the first and most important step towards creating a safe work environment.

            Calling out their attention against possible hazards, each one of us must address our colleagues intentional and
            unintentional safety deviations which can lead to possible hazards.

            We must be very systematic to address the unintentional unsafe behaviours through,
               1.  Effective Safety induction training
               2.  Create more awareness - Relevance Hazards and risks explained to the workers before start of works
               3.  SOP's – Defining & Explaining safe work methodologies
               4.  Carry out Frequent safety walk / inspection / Audit

            Also, to address the Intentional / casual approach which can put self or others into the risks,

               1.  Encourage Identify and report unsafe acts by any colleagues of possible Hazards
               2.  Use Effective system to address the misbehaviour with governance procedures. We shall encourage the
                   reporting the unsafe conditions and reward personnel's works / make efforts towards resolving the unsafe
                   condition as a proactive.

            Act cautiously with a “maybe/doubtful” mindset”.

            Always be open and act cautiously, identify all the hazards and remove before start of the works through HIRA or
            JSA. Safety kaizens shall be used as effective to eliminate unsafe conditions and prevent unsafe act by introducing
            Poka-Yoke techniques. It's our responsibility to provide safe work environment to everyone.

            Please refer the Special Safety bulletin / Notice release to address the above issue (11 Pointers ) before start of any
            work.

            Always keep the 11 pointers in your pocket and read out in the morning / shift starting toolbox talks at least once in
            a week.
