Mandated Reporting Procedure: Long Term Care Ombudsman
If the suspected or alleged abuse or neglect occurred in a long-term care facility, the following
shall occur:
If the suspected abuse results in serious bodily injury, a telephone report shall be made to the
local law enforcement agency immediately, but also no later than within two hours of the
mandated reporter observing, obtaining knowledge of, or suspecting the physical abuse,
AND
a written report (SOC 341) shall be made to the local ombudsman, the corresponding licensing
agency, and the local law enforcement agency within two hours of the mandated reporter
observing, obtaining knowledge of, or suspecting the physical abuse.
WIC Section 15610.67. "Serious bodily injury" means an injury involving extreme physical pain,
substantial risk of death, or protracted loss or impairment of function of a bodily member, organ, or
of mental faculty, or requiring medical intervention, including, but not limited to,
hospitalization, surgery, or physical rehabilitation.
If the suspected abuse does not result in serious bodily injury, a telephone report shall be
made to the local law enforcement agency within 24 hours of the mandated reporter
observing, obtaining knowledge of, or suspecting the physical abuse,
AND
a written report (SOC 341) shall be made to the local ombudsman, the corresponding licensing
agency, and the local law enforcement agency within 24 hours of the mandated reporter
observing, obtaining knowledge of, or suspecting the physical abuse
If the suspected abuse is other than physical (abandonment, abduction, isolation, financial
abuse, or neglect), telephone the Long Term Ombudsman or Local Law Enforcement agency
immediately or as soon as practicably possible
AND
Submit a written report (SOC 341) to that entity within two (2) working days.
In any reportable situation, call your QA Coordinator and submit a written report (SOC 341)
within the applicable time frames above
IMPORTANT: If you find a consumer in a life threatening situation, call 911 and report the
situation to the Police, Fire Department and/or Emergency Medical Personnel
