Mandated Reporting Procedure: Adult Protective Services
If the suspected or alleged abuse (abandonment, abduction, isolation, financial) or neglect
occurred anywhere other than a long term facility, the following shall occur:
Call the Adult Protective Services (APS) and your QA Coordinator immediately or as soon
as practicably possible.
AND
Submit a written report (SOC 341) to Adult Protective Services (APS) and to your QA
Coordinator within two (2) business days.

Reporting Issues of Health and Safety or Violations of Rights
If you become aware of a situation potentially dangerous to consumer’s health and safety or
become aware of a possible violation of any legal, civil or service rights:
Immediately call your QA Coordinator or as soon as practicably possible
AND
Email/fax a written narrative of the situation within 24 hours, or the next business day.
IMPORTANT: If you find a consumer in a life threatening situation,
call 911 and report the situation to the Police, Fire Department
and/or Emergency Medical Personnel
