Proxy Determination

• Happens during Initial Contact and should be reconfirmed before the
survey meeting begins.

• Questions must be asked directly of the person receiving services.

• It is ultimately the surveyor's decision whether the person will be able

to answer any questions in Section I validly.

• Communication is not a reason to consider responses invalid.

Question to surveyors:

• What do you do if a family member or staff tell you the person will not

be able to answer questions?

• Do surveyors have any other questions about the Proxy Determination
process?
