Benefit eligibility criteria

The following are GESMN’s classifications of employees and the benefits each group is eligible
for as of 1/1/2020:
Full-time regular employees consistently scheduled 35+ hours per week
• Eligible for all GESMN benefits which include medical, HSA, dental, vision, life, LTD, STD, FSA,
CIGNA plans, legal, pet insurance and Employee Assistance Program
• Eligible on the first of the month after 30 days of full-time regular status
Part-time regular employees consistently scheduled 30-34+ hours per week
• Eligible for GESMN benefits which includes medical, HSA, dental and Employee Assistance
Program
• Eligibility on the first of the month after 30 days of part-time regular status
Part-time variable employees who average 30+ hours per week
• Eligible for select voluntary GESMN benefits including medical, HSA, dental and Employee
Assistance Program if average hours total 30+ hours per week after one of the following
periods:
• New hires are measured 12 months after date of hire
• All employees are measured annually each October
Part-time variable employees (all other classification of employee)
• Eligible for select voluntary GESMN benefits including dental and Employee Assistance
Program
• New hires eligible on the first of the month following 30 days of hire date
