When benefit coverage begins
For new hires, the following benefits begin on your date of hire. For
benefit changes, the benefits begin on the benefit event effective date,
providing you elect coverage during and enrollment period and meet the
plan’s eligibility requirements.
› Medical plan (includes vision and pharmacy)
› Dental plan
› LiveWell wellness program
› Section 125 and flexible spending accounts
The following benefits automatically begin on your first day
of employment:
› Employee assistance program
› Group life and accident insurance
› Long-term disability insurance
› Student loan resource
› Workers’ compensation benefits
Coverage begins for other benefits as listed below:

Basic Retirement Savings Plan First of the month following date of hire
Voluntary 401(k) Retirement Savings Plan Pay period following your enrollment date
Supplemental Term Life and Accident Insurance; First of the month following date of hire, or first of
Spouse Life and Dependent Life Insurance the month following policy issue date if elected
outside of new hire guarantee issue period
Short-Term Disability Benefit 90 days after date of hire
Supplemental Long-Term Disability Insurance Date the policy is issued
Cancer Insurance Date the policy is issued
Property and Casualty Insurance Date the policy is issued
Identity Theft Protection First of the month following your enrollment date
Pet Insurance The date you enroll with the vendor
