Second medical opinion service
2nd MD is a free resource available to all medical plan participants.
If you are dealing with a medical issue or if you are considering surgery,
this resource allows you to receive a second medical opinion from
world-class leading doctors. 2nd MD takes care of collecting your medical
records and connecting you with the right doctor to review your health
issue. This resource may lead you to a better medical outcome and
help save you money. See the Benefit Contacts section for how to
contact 2nd MD.
Vision benefits summary
Vision coverage is provided by UnitedHealthcare and is included when
you enroll in the medical plan. Vision benefits include an eye exam,
lenses and frames once every 12 months. Dependents under the age of
13 have access to a broader pediatric vision benefit. See the pediatric
vision summary posted on the HR intranet site.
If you use an in-network provider, the copay for a vision exam is $15
and the copay for materials is also $15. The materials benefit includes
glasses or contacts lenses. The contact lens benefit includes the fitting
and purchase of one pair of contact lenses, or up to six boxes of
formulary disposable lenses.

Pharmacy benefits summary
Pharmacy coverage is provided by Express Scripts and is included when
you enroll in the medical plan. To receive network benefits, you must
present your Express Scripts prescription identification information at
participating pharmacies. If you fill a prescription at an out-of-network
pharmacy, you will be required to pay the entire cost of the drug and file
a claim for reimbursement (copayment and coinsurance apply). The plan
also includes a program to save you money when you fill maintenance
medication. See important details below.
Prescriptions for maintenance medications
We participate in Express Scripts’ Smart90 Rx program to help you save
money when filling prescriptions for maintenance medication. With this
program, you can fill a 90-day supply for less than three copayments.
Maintenance medications are long-term medications you take regularly
(i.e., for high blood pressure, cholesterol, allergies, birth control and
diabetic supplies). To determine if your prescription is considered
maintenance medication, contact Express Scripts or view this information
at Express-Scripts.com.
To benefit from the Smart90 program, ask your physician to write your
prescription for a 90-day supply and fill it at your local Walgreens retail
pharmacy or through Express Scripts’ mail order program. If you fill your
maintenance medication at a different pharmacy, or for a different
quantity, you will be charged 100% of the cost for the medication.
