Coverage begins for these benefits on a new hire’s first day of
employment, or on the benefit event effective date (for benefit changes),
providing you elect coverage for plans that require an election and meet
the plan’s eligibility requirements.

Basic Retirement Savings Plan First of the month following date of hire
Voluntary 401(k) Retirement Savings Plan Pay period following your enrollment date
Supplemental Term Life and Accident Insurance; First of the month following date of hire, or first of
Spouse Life and Dependent Life Insurance the month following policy issue date if elected
outside of new hire guarantee issue period
Short-Term Disability Benefit 90 days after date of hire
Supplemental Long-Term Disability Insurance Date the policy is issued
Cancer Insurance Date the policy is issued
Property and Casualty Insurance Date the policy is issued
Identity Theft Protection First of the month following your enrollment date
Pet Insurance The date you enroll with the vendor
