Monthly Premiums


              Employees  and  spouses  can  earn
              wellness  incentives  that  reduce  their

              medical  premium.  Incentives  can  be

              earned  by  being  tobacco-free,  by

              completing  a  biometric  screening  and  a
              health  assessment,  and  by  participating

              in the LiveWell wellness program.
