Q5. (a) Mention 6 general questions in a proposal form
for non-industrial risks ?

Ans. Six general questions are :
(i) Name and Address of the Proposer.
(ii) Whether the proposer has complied with all statutory
rules pertaining to premises and business activities.
(iii) Whether the premises have boundary/fencing.
(iv) Security or Safety arrangement.
(v) Details of systems for prevention of fire explosion
etc.
(vi) Past insurance history.

(b) Mention 6 questions relating to schools?
Ans. (i) Number of students and their age group.

(ii) Whether hostel facility is provided. If yes, number
of rooms, number of inmates.

(iii) Whether canteen facilities are provided in the
institution/hostel. If yes, whether they are
hygienically maintained.

(iv) Whether cover needed against risks associated with
food and beverages served in the institutions.

(v) Details of other facilities provided, i.e (a) Indoor
Games (b) ) Outdoor Games (like Mountain
climbing, Hand gliding, Horse riding, Swimming etc
and whether such games are taught under the
