Marine cargo insurance questionnaire covers the following information: –

a. Name of the shipper or client

b. Description of goods

c. Method and type of packing

d. Voyager mode of transit

e. Cover required and conditions of insurance

f. Name of the steamer, railway receipt etc

g. Sum to be insured

h. Past came experience

Cover note

Cover note is a document issued in advance of the policy. After the negotiations

are completed it may take some time for issuance of the policy. During the

intervening period the cover note is issued as evidence of protection temporarily.

Once the policy is issued the cover note has no value. Sometimes cover note is

issued provisionally while the actual rates are being determined. The cover note

is not stamped.
