non early death claim. The requirements for settlement of the two types of claim are
different as detailed below.
Claim documents and forms and settlement procedure :
We will discuss in this section the insurance documents necessary at the time of the
final payment. The final payment may relate to the maturity or death claim payment.
The documents required for payment of maturity claim :
1. Age proof, if age is not admitted.
2. Original policy document for cancellation.
3. In case assignment is executed on a separate paper, that document has to be
surrendered.
4. Discharge form duly executed.
5. Indemnity bond in case the policy document is lost or destroyed, duly executed
by the policy holder and a surety of sound financial standing.
The documents required for payment of a death claim.
1. An intimation of death by the nominee or a near relative.
2. Proof of age if not already admitted.
3. Proof of death.
4. Doctor’s certificate who attended the deceased during his last illness.
