Lowest-cost plans for the Rx Drugs that you take

                                                                 November-December 2018 All of 2019

Wellcare Value Script PDP NA* $602
               (888) 293-5151

Aetna Medicare Rx Select PDP $115 $630
               (833) 856-5680

               SilverScript Choice PDP $102 $684
               ((866) 235-5660

If you want to be in the lowest-cost plan for the rest of this year and also to be in the lowest-cost plan
for next year, you can enroll in the SilverScript Choice plan now and then in December call the Wellcare
Value Script plan to enroll for the 2019 plan year. If you choose to do this, your total estimated Rx drug
               costs for the next 14 months will be $704 ($102 + $602).

In most cases it’s fairly easy to switch drug plans – you simply call the plan you want to enroll in and tell
them the date you want your coverage to start (January 1). You do not need to dis-enroll from your
current plan because when your new plan notifies Medicare that you will be enrolled as of January 1,
then Medicare will dis-enroll you from your old plan at the end of December. And since CVS is a network
pharmacy in both plans, you should not have to get new prescriptions if you switch plans.

The other option is that if you prefer to enroll in a plan now that has the lowest costs for the next 14
months and avoid having to switch plans in December, you should enroll in the Aetna Medicare Rx
Select plan. If you do this, your total estimated Rx drug costs for the next 14 months will be $745 ($115 +
$630). There’s a separate attachment showing explaining this further.

Finally, there are a couple of things to keep in mind as you make the transition to Medicare. On
your initial visits to your doctors after January 1, show the office staff your new enrollment
cards for Medicare A&B, your Medigap policy and your Part D stand-alone plan. That will help
to avoid any billing confusion. Also, you’re entitled to a free “Welcome to Medicare” physical
(see page 11 of the evaluation), but you’ll probably have to request it.

                                                   Sincerely,

                                                   David Armes, CFP®
