               Dear Mr. Heald:

               Your Medicare evaluation is enclosed. Please check your client information on page 5 of the
               evaluation to make sure it is accurate. Except for the prescription drug costs, the premiums
               shown on page 3 are estimates for the entire 2020 plan year, even though this year you will be
               enrolled for only eight months.

               Your actual costs will be greater than those shown because as you mentioned, Dr. Konda has a
               concierge practice and you pay him a separate fee. Your other physicians all accept Medicare-
               approved payments, as indicated in Appendix A1.

               Based on the information on his web site, shown in Appendix A1, Dr. Konda has opted out of
               Medicare (some concierge practices opt out of Medicare while others do not). Even though a
               physician opts out of Medicare, he or she may refer you other providers such as labs, imaging
               services, and physical therapists that do accept Medicare. In any case, it is a good idea to verify
               when Dr. Konda refers you to another provider for a service that it will be covered by Medicare.
               Additional information on the opt-out process is in Appendix A2.

               Another possible reason your costs may be greater than those shown is that if your income
               exceeds a certain level, you may pay higher Part B and Part D premiums. Appendix B explains
               the basics of how these higher premiums are determined.

               There are three steps you will need to complete before May 1:

                       1)  Enroll in Part A and Part B with an effective date of May 1, 2020. Because you have
                          already started receiving your Social Security benefits, you will be automatically
                          enrolled in Part A and Part B, and you may have received your Medicare card by
                          now. If not, you can call Social Security at (800) 772-1213 to inquire about your
                          enrollment status. Your Part B premiums will be deducted from your monthly Social
                          Security checks.

                       2)  Acquire a Medigap policy.

                       3)  Enroll in a Medicare prescription drug plan, also called a stand-alone drug plan.
