Life and Accidental Death and
Dismemberment (AD&D) Insurance
The Life and AD&D insurance plans are administered by Unum. These plans provide protection for you and
your dependents. These coverages are available to full-time employees working 30 hours or more per week.

Beneit Amount 2× annual earnings up to $700,000
Guarantee Issue Amount 2× annual earnings up to $700,000
Age Reduction At age 70, beneits will reduce by 50%
Tax Considerations/Imputed Income
As a result of offering employer paid group term life insurance, IRS Code 79 requires that life coverage over
$50,000 is taxed annually on the imputed income. Imputed Income is the basis for the amount of tax owed
for coverage over $50,000 provided by an employer. The calculated cost of imputed income is reported as
part of your wages each pay period. Employees pay what may be due when iling their federal income tax
