How to Open an HSA Bank Account
If you enroll in the High Deductible Health Plan, you should open an HSA account through OptumBank. You
will open your HSA though Optum Bank. Each company has been assigned a unique URL link for you to use to
open an account. The URL assignments are as follows. For your convenience, scan the QR code with your smart
phone to visit the website directly.

Note: You do not need to have a UHC member ID number to open an Optum account. You do need a Social Security Number.
After opening an account, Optum will send three separate mailings to your home.
