Medical Coverage
New Spouse Eligibility Rules
Due to the increased cost in healthcare, F+W is changing the deinition of an eligible
spouse effective January 1, 2016. Full-time working spouses or domestic partners
who have access to medical coverage through their employer will no longer be
eligible for F+W’s medical plan. If your spouse or domestic partner is not employed
or does not have access to coverage through their employer due to the below
reasons, there is no change to their eligibility.
X My spouse is not employed
X My spouse works part-time
X My spouse is self-employed and does not have access to medical coverage
X My spouse is not eligible for coverage at their employer
X My spouse is currently in their eligibility waiting period with their employer
X My spouse’s employer does not offer medical coverage
X My spouse is on Medicare and does not have access to any employer program
If one of the above scenarios applies, your spouse can remain enrolled in F+W’s
medical plan, provided they complete the Spouse Veriication Form and return it
to human resources. This change in eligibility only applies to the medical plan. All
spouses or domestic partners remain eligible for the dental, vision, life, and the new
coverage offerings for 2016. This provision also does not affect the deinition of an
eligible child.
As stated, all employees who want to keep their spouse enrolled in the plan will need
to complete a Spouse Veriication Form and return it to human resources. If you do
not return a completed form by the end of open enrollment, your spouse’s coverage
will end on December 31, 2015. For additional information, please see the
