2015 Medical Plan Options
Medical Plans
Choosing the right medical plan is probably one of the most important
decisions you can make for yourself and your family members.
ASARCO LLC offers two different health plans to meet your speciic
needs. UMR administers both the HRA and the PPO plan. Both plans
offer access to a Preferred Provider Organization (PPO) covering
physicians, hospitals, health care centers, laboratories, pharmacies, and
other health care providers. Both plan designs offer in-network and
out-of-network coverage. You will have lower out-of-pocket costs on
services you receive if you visit in-network providers.
Preventive care is covered in-network at 100% in both plans. Preventive
care services are not charged against your deductible or coinsurance
responsibilities. Note, preventive care services are only covered in-
network.
The HRA Plan has a higher deductible and out of pocket maximum
that can be offset with wellness incentives. If all wellness incentives
are earned by the employee and/or spouse the HRA Plan matches the
PPO Plan deductibles and out of pocket maximums.
Plan participants who meet Gold Vitality status in 2015 will pay a
lesser premium in 2016 compared to those that do not meet Gold
status.
