Tobacco Cessation Program

Tobacco Cessation Program

ALCO Management rewards employees and their spouses who are

enrolled in the medical plan and do not use tobacco products or products
which deliver nicotine by offering a lower monthly medical premium.

If you or your spouse use tobacco products or products which deliver
nicotine, and did not participate in the tobacco cessation program you will
not be eligible for lower monthly medical insurance premiums beginning
January 1, 2017.

Completing the tobacco cessation program is deined as:

ƒ Completing the health risk assessment or enrolling in “Your Health
First”, selecting Tobacco Cessation, completing the eight week
program, and being nicotine/tobacco free for two months

Register online at www.mycigna.com; complete the health risk assessment
or enroll in “Your Health First” and select tobacco cessation.

You will be required to certify you are a non-tobacco user or have not
used products which release nicotine or you participated in the tobacco

cessation program in order to receive the premium discount beginning
January 1, 2017.

Please complete the tobacco/non-tobacco user certiication form and
return to Cindy Phelan by December 31, 2016.
