Voluntary Beneits








Critical Illness, Group Accident, and Hospital Insurance

Important! You may only elect coverage under a voluntary group benefit plan during open
enrollment by accessing the enrollment page at www.DANbenefitsPLUS.com.

If you’re concerned about the inancial impact large medical expenses can have on you and your family if you or they
are diagnosed with a serious illness or sustain a serious injury, consider enrolling for Criical Illness, Group Accident, or
Group Hospital Insurance coverage from AFLAC.

The Criical Illness group plan pays a lump-sum beneit if a covered person is diagnosed with a heart atack, stroke,
cancer, or needs coronary artery bypass surgery or a major organ transplant. It can help cover out-of-pocket medical
expenses for treatments that aren’t covered by your medical plan such as deducibles and coinsurance.


You may choose group Criical Illness insurance as follows:
X Employees—can be elected in increments of $5,000 up to $30,000

X Spouses—can elect $5,000, $10,000 or $15,000, but elecion cannot exceed 50% of the employee coverage amount
X Children—coverage available up to a maximum of 50% of employee only elecion

In addiion, the Group Accident insurance plan helps ofset your out-of-pocket expenses in the event of an accident,
such as copays, coinsurance, and other medical expenses. You may elect the following Group Accident insurance
(without any need to provide proof of good health).
X Employee—up to $50,000
X Spouse—$25,000

X Child(ren)—$5,000
X $50 wellness credit if received annual physical

The Hospital Insurance program pays a cash beneit to you for covered hospital stays, outpaient surgery costs, intensive care
stays, and post hospital skilled nursing facility care. This cash payment can be used however you choose.
