Disability Beneits


Short Term Disability STD Schedule of Beneits Based on Length of
Short-Term Disability (STD) provides you with a percentage of your weekly Service
income while you are out of work following a non-work related accident or Number of Weeks
illness. DAN ofers a STD plan at no cost to you. Those eligible will receive Years of Service Covered (includes
eliminaion period)
an STD beneit equal to 100% of their weekly earnings and are eligible to Less than 1 year 4 weeks

STD beneit is ofset by any FMLA or state mandated disability beneits 1 year but less than 2 8 weeks

Long Term Disability
Long-Term Disability (LTD) protects your income by providing you with Long Term Disability Example
a percentage of your income while you are disabled. DAN provides an Opion Opion
LTD beneit at no cost to you. In the event of an illness or accident that A— B—
requires a lengthy recovery, you may be eligible for a tax-free beneit Company Employee

equal to 60% of your monthly salary up to a new maximum of $20,000
per month. You are eligible to receive this beneit unil you reach Social Employee Salary $50,000; tax bracket:
