acquisition brings some changes to our beneits program. It’s never
been more important for you to take considerable time in educating
yourself about your beneit choices. We encourage you to read this
guide and share this information with your family as you determine
which programs best meet your needs.

Additional details about pricing are included with the enrollment
materials.
You have from October 20 through November 3 to participate in open
enrollment. You must complete and return your enrollment form to
Human Resources by November 3 to have coverage in 2015. Keep in
mind, open enrollment is your annual opportunity each year to make
changes to your beneit options. You cannot change your coverage
during the plan year unless you experience a qualifying life event.
