Open Enrollment
To: All U.S. Colleagues
From: Jeffrey S. Davis, CEO and President
Subject: 2016 Benefit Open Enrollment
Scheduled for 11/2/15 through 11/13/15
Date: October 19, 2015
At Perficient, we are committed to offering you affordable, competitive benefit options. The packages are designed
to be flexible and balanced in meeting your individual needs and at the same time sustainable and cost effective for
Perficient. Because the health of you and your loved ones is so important, I want to ensure that you have the proper
context around variables that impact our options and coverage.
The Affordable Care Act, combined with other market conditions, continue to drive healthcare costs higher. As the
costs and complexities increase, the delicate balance of offering the right benefit options at the right time and at the
right prices becomes even more challenging for companies large and small. So while I’m pleased to share that
Perficient will continue to subsidize the majority of the cost for all colleagues, our personal premiums will increase
for 2016.
We benchmark our offerings and premiums against a number of our competitors every year and we’re confident
that Perficient’s 2016 plan options provide you competitive and valuable coverage.
There are some important things to know about this year’s open enrollment process:
The open enrollment portal will be open November 2 through November 13 .
If you are happy with your current benefit choices you WILL NOT have to re-enroll in benefits for 2016.
Your current choices will roll over automatically into the new plan year.
If you contribute to a Health Savings Account (HSA) or one of our Flexible Spending Accounts, and you wish
to continue in those programs, you WILL have to re-enroll during open enrollment.
We will manage the open enrollment process through our Oracle system.
You will continue to have a choice between two medical plan options with Anthem: a PPO or Health Savings
Account (HSA).
Our dental, vision, long term disability/accidental death and life insurance options will not change.
There are only four changes that you will see in our 2016 plan design:
We are adding a fourth tier to our specialty pharmacy copay coverage.
We are implementing required participation in our current pharmacy mail-order option for refills of
medications that are taken on a routine or regular basis. It is required if you receive more than two refills.
Because of regulatory changes, our HSA plan deductible is increasing; however, the family out-of-pocket
maximum is decreasing.
We are adding an eligibility requirement for spouses.
A Q&A document has been prepared to ensure that you have the necessary information to assist you in
understanding what these changes mean and how they may affect you.
Remember, open enrollment is the one time each year that you can actively make changes to your Medical,
Dental, Vision and FSA plan elections unless you have a family status change during the year .
If you have any questions regarding your benefit programs, I encourage you to reach out to the Human Resources
Department at Benefits@Perficient.com.
Sincerely,
Jeffrey S. Davis
