Medical Plan Options The Nortek health plans are account-
based health plans, which means you

The Kaiser HMO, or Health Maintenance Organization, is a speciic
group of healthcare providers afiliated with the Kaiser Permanente
network. In the HMO plan, you must designate a Primary Care Physician
(PCP) when you enroll in the plan. You will only receive beneits and
coverage when you obtain care through a Kaiser Permanente physician
or facility (except in the event of an emergency). More details about your
