Qualifying Events

Eligibility A qualifying event is a change in your personal life which may impact
your eligibility or dependent’s eligibility for beneits.


Examples of some qualifying events include:

X Change of legal marital status (i.e., marriage, divorce, death of
spouse, legal separation)

X Change in number of dependents (i.e., birth, adoption, death of
dependent, ineligibility due to age)
X Change in employment or job status


It is your responsibility to notify your facility Administrator within
30 days of the qualifying life event. Failure to notify your facility
Administrator within 30 days will prohibit your ability to change your
beneits election(s).


Eligible Employees


You may enroll in the Americare Employee Beneits Program if you
are a regular full-time employee who is actively working a minimum
of 30 hours per week. All Guardian plans are available to employees
working 20 or more hours per week (see beneit details for exceptions).


Dependent Eligibility

Your legal spouse, children, or legal guardianship dependents are

eligible for coverage in the Americare beneit program. Your dependent
children can be covered until the age of 26 regardless of student or
marital status. Children after age 26 with a permanent or total disability
can remain covered under Americare’s plans for as long as you are an
active and eligible employee.
