a $10,000 beneit. All dependent child elections are guaranteed issue
without needing to submit proof of good health. If your spouse or
dependent passes away while covered by this plan, the beneit is paid
to their designated beneiciary (or beneiciaries). Remember to elect a
beneiciary for this coverage.
