The Hartford Company Paid Short Term Disability

Policy #696980 The company provides employer paid short term disability based

X Under one year of service—not eligible

X One to four years of service will receive up to six weeks of company
paid Short Term Disability

X Four years and above will receive up to eight weeks of company paid
Short Term Disability

You will also have the option to purchase additional coverage.

Voluntary Short Term Disability

In the event that you want to protect yourself for a full 12 weeks before
Long Term Disability would become active due to an extended disability,
you have the option to purchase voluntary Short Term Disability to
help close the gap with the company paid Short Term Disability. The
company paid Short Term Disability plan will kick in irst and based
upon your tenure, your voluntary Short Term Disability plan would
start after that ends. The plan will provide 60% of your pre-disability
earnings.

Company Paid Long Term Disability

The company provides Long Term Disability insurance to offer you
inancial assistance in the event you are unable to work for an extended
period of time. The plan covers 60% of your pre-disability earnings to
a maximum monthly beneit of $5,000 once you have been out of work
for 90 days.
