Eligibility
X Employees—all regular full-ime and part-ime employees working at Eligibility and
least 21 hours per week Enrollment
X Spouses—includes same-sex or opposite sex domesic partners subject
to program rules (medical, dental, and vision only); must complete
Domesic Partner Ceriicaion

X Child(ren)—children up to age 26

Important Notes
X Beneit plans for same-sex spouses will be provided according to the
same federal tax rules as they are for opposite-sex spouses; if you wish to
enroll your same-sex spouse for 2017, simply elect married and list your
spouse as an eligible dependent

X If you are enrolling a domesic partner for coverage for the irst ime you
will need to provide an aidavit validaing your domesic partner status;
visit www.DANbeneitsPLUS.com to download the Domesic Partner
packet which includes the aidavit


Making Changes During the 2017 Plan Year
Under IRS rules, your beneit elecions must remain in efect unil
the next plan year which starts on January 1, 2018 unless you have a
qualifying event during the year. You must register your changes by
visiing www.DANbeneitsPLUS.com or by calling the beneitsPLUS soluions
center within 31 days of the qualifying event.

In accordance with Federal Regulaions, Dentsu Aegis Network is required to send employees informaion
regarding the beneitsPLUS Flexible Beneits Plan. Please review these noices on www.DANbeneitPLUS.
com as they contain important informaion relaing to Health Care Exchanges, Special Enrollments, Noice
of Privacy Pracices (HIPAA), and more. When you begin your enrollment process the system will require
you to acknowledge receipt of the Annual Noices.
