2018 CareHere Employee


Wellness Program



Participation in the 2018 Wellness Program is voluntary. Covered
medical employees and spouses will have the option to complete the
2018 requirements in order to be eligible for $600 health reimbursement
arrangement (HRA) dollars (maximum $1,200 if both employee and
spouse participate) which will be placed into your CareHere health
reimbursement account (HRA) for the 2019 plan year. More details will
be provided concerning the 2018 wellness program requirements in

December.


Tobacco Surcharge

CareHere charges an additional $23.08 per pay period for medical
premium; $46.15 per pay period if both you and your spouse are enrolled
and use tobacco. During open enrollment you will receive the CareHere
Employee Statement of Tobacco Usage form via email. Please complete
and return per the email instructions. If you are a current tobacco user,
you must register for the tobacco cessation program by December 31,
2017 and must complete the program by May 31, 2018.


Employees hired after December 1, 2017 will not be subject to the
surcharge until the 2019 plan year.
