HUMANA GO365
WELLNESS PLAN
Humana provides you and your eligible family members with access to the
Go365 wellness program. You do not have to be enrolled in one of our medical
plans to participate. This program allows you and your family members to
earn points for completing health activities. Your points can convert into
Bucks worth up to $300 per plan year. myNEXUS also benefits when you earn
points through the Go365 program. Humana has agreed to reduce our renewal
rates based on the percentage of myNEXUS employees who achieve a status of
Silver or better. Download the Go365 App to learn how to start earning points.
It Pays to Engage
If you (and your covered dependents at least 18 years of age) are enrolled in
one of the myNEXUS medical plans, you have the opportunity to lower your
per pay period medical plan premium if you achieve a Go365 engagement
level of Silver or Gold as outlined below. The wellness premium rebate will be
added to your taxable income each pay period.
Silver Gold
Monthly $25.00 $75.00
Per Pay Period $11.54 $34.62
Plan Year Savings $300.00 $900.00
If you and your eligible dependents achieve Silver or Gold status by April 30,
2018, you will start to receive your wellness credit as of the first pay period
following May 1. If you have not achieved Silver status by April 30 or if you
would like to improve to Gold status, you are encouraged to do so between
May 1 and December 31. myNEXUS will update Silver and Gold status levels
based on your achievement as of July 31, October 31 and December 31. Your
wellness credit will begin or change the following pay period. If you achieve
Silver or Gold status by December 31, you will also qualify to receive the
wellness credit for the new plan year beginning May 1, 2019 assuming there
are no changes to the May 2019 medical plan.
