What Happens If I Terminate Employment?
If you terminate your employment with Fontbonne University during
the plan year, your right to beneits will be determined in the following
manner:
You will remain covered by insurance up to the end of the month in
which you terminate, provided premiums have been paid prior to your
termination of employment.
Under Federal law, if you, your spouse, and/or covered dependents
lose coverage under this plan, then you, your spouse, and/or your
covered dependents may be entitled to continuation of healthcare
coverage through the Consolidated Omnibus Budget Reconciliation
Act (COBRA). Additional information about your continuation
coverage rights is provided at the time of, or shortly after, termination.
You will still be able to request reimbursement for qualifying
dependent care expenses for the remainder of the plan year from
the balance remaining in your dependent care account at the time of
termination of employment. However, no salary contributions will be
made on your behalf after you terminate.
You may elect to continue your participation in the Health Flexible
Spending (Reimbursement) Plan for the remainder of the plan year.
If you do so, you must continue to make any required contributions
to the plan. If you do not wish to continue participating, your
participation will cease and no further salary contributions will be
contributed on your behalf.
If your participation in the Health Flexible Spending
(Reimbursement) Plan ceases, you will be able to submit claims for
healthcare expenses incurred prior to your date of termination. You
must make requests for reimbursement no later than 60 days after
your date of termination.
