Eligibility and Enrollment
If you are hired as a full-time employee working 30 or more hours per week, then you are
eligible for beneits the irst day of the month following your date of hire.
If you are hired as a part-time, variable-hour, or seasonal employee and are expected to
work less than 30 hours per week, then your hours will be considered over a 12-month
measurement period. At the end of the 12-month measurement period, if it is determined
you have worked an average of 30 hours per week, then you will be offered beneits for the
following 12-month period, deemed the stability period.
You may enroll your eligible dependents for coverage once you are
eligible. Your eligible dependents include the following.
X Your legal spouse, or domestic partner
Z Provided they meet the requirements under the Working
Spouse Rule (see page 5)
X Your children up to age 26
If you do not enroll for insurance within your irst 30 days of
employment with F+W you will need to wait for the next open
enrollment. Once your beneit elections become effective, they remain in effect until the end
of the year. You may only change coverage within 30 days of a qualiied life event.
Qualified Life Events
Elections (including waivers) are binding for the entire calendar year. During the plan’s open
enrollment time period, which generally occurs in late Fall, you will have the opportunity
to change your elections for the following calendar year. The only way you can change your
election mid-year is if you experience a qualiied life event. A qualiied life event will allow
you to change your coverage election. Examples of a qualiied life event include the following.
X Marriage X Change in employment status of
X Divorce or legal separation employee, spouse, or dependent child
X Birth of your child X Qualiication by the Plan Administrator
X Death of your spouse or dependent child of a child support order for medical
coverage
X Adoption of or placement for adoption X Entitlement to Medicare or Medicaid
of your child
