Preventive Care
Preventive care can help you and your family avoid
potentially serious health conditions and/or obtain
early diagnosis and treatment. Generally, the sooner
the doctor can identify and treat a medical condition,
the better the outcome.
If you are enrolled in a CommScope UHC medical plan,
you and your covered family members have access to
in-network preventive care covered at 100% without
any out-of-pocket cost.
Preventive Care Guidelines For Children
Recommended preventive care services for children
will vary based on age and may include some of the
following:
„ Age-appropriate well-child examination
„ Age-appropriate immunizations
„ Hearing screening
„ Autism screening
„ Vision screening by primary care doctor
Preventive Care Guidelines and Counseling
For Adults
Recommended preventive care services for adults may
include the following:
„ Wellness examinations
„ Breastfeeding primary care interventions,
counseling, support and supplies
„ Cholesterol screening (age 40-75)
„ Genetic counseling and evaluation for BRCA Testing,
and BRCA lab testing
„ Blood pressure screening
„ Immunizations
„ Obesity screening and counseling
„ Tobacco cessation, screening, behavioral counseling
„ Mammography screening
