7.2.13 Boom & Gantry Anti Collision
a) KFTL cranes came with a basic Boom Anti- Collision wire system to protect the boom from striking vessel accommodation etc.
b) Many of these systems are either missing or inoperable.
c) Trent was advised that such a collision did take place in recent times, whereby a crane struck the bridge of a vessel, knocking the ships radar to the ground. It was further advised that the vessel in question missed its transit slot through the Panama Canal, and that a significant claim had been lodged against KFTL.
d) Such system failures represent a significant monetary, safety and operational risk to KFTL.
e) Boom limit switch case and conduit badly corroded, which will ultimately end in sensor/ switch failure.
f) Failure to detect boom position and end stop – can result in a serious incident.
g) Gantry crane collision prevention device not working.
