b) Trolley rails on Cranes Q-P-M are in poor condition, with serious defects at the boom hinge, where the short rails are constantly being repaired. Other cranes are also suffering from the same problem.
c) Trent has developed some robust procedures and redesign for corrective action.
d) It would be Trent’s recommendation to proceed without delay, in replacing the short rails on these three cranes, using high quality short rails, supplied pre-machined and ready for installation, by a specialist European rail supplier. The remaining cranes can be budgeted for the same remedial work, over time, as required.
e) This item is listed in the action plan of this report.
7.2.21 Rail Span Issues
a) On Cranes N-M-L-K, the wheel span is way out of tolerance with the FEM and other recognised international standards, and as a consequence, severe gantry wheel flange wear is being experienced.
b) A geometrical survey conducted during December 2017 and January 2018 by KFTL show the following results:
• The actual span of the gantry rails is within tolerance, only varying by + 1 mm. to - 4 mm.
• Crane rail span is out of tolerance varying by as much + 51 mm. in the worst case on these four cranes.
• In the case of these four cranes, the adjusting devices (large bolts) installed on the main and often secondary equalisers have not been provided with this design.
• These devices should be retrofitted to allow adjusting of the equaliser hence the crane wheel span.
• This recommendation is listed in the action plan of this report. 7.2.22 Risk Awareness
a) Those items above, marked with   are considered by Trent to present a safety risk to KFTL.
b) Some of the items were discussed, risk assessed and profiled, and can be reviewed in Section 13 of this report, entitled ‘Risk Management & Profiling’.
c) Due to time constraints on-site and the sheer number of issues at KFTL, the remaining items need to go through a proper risk assessment in order to properly quantify the risk itself and ensure they are correctly prioritised in the KFTL action plan.
