            Important reminders about qualified medical expenses:
            • As the HSA owner, you are ultimately responsible for determining whether a healthcare expense is
              eligible for reimbursement from your HSA.
            • Salisbury Bank has provided these lists to serve as a quick reference, however it is not intended to be
              comprehensive and they are offered with the understanding that Salisbury Bank and Trust Company is not
              offering tax advice. For more detailed information, please refer to IRS Publication 502, titled: “Medical and
              Dental Expenses (Including the Health Coverage Tax Credit)” Catalog Number 15002Q. Publications can
              be ordered directly from the IRS by calling 800.TAX.FORM.
            • If an HSA expenditure is not used for a qualified medical expense, you will be required to pay
              income tax and a 20 percent penalty on the amount used. (The 20 percent penalty tax does not apply
              to payments made after the date you are disabled, reach age 65 or die.)
