Open Enrollment
The Open Enrollment period allows for employees to add or change their benefits and
their coverage. Applications for the various programs may be submitted during this
period. Changes, additions, and other elections made during this period will take effect on
the effective date after the period closes. Changes are permanent until the next Open
Enrollment Period, unless there is a qualifying change in your status (see Special
Enrollment).
Special Enrollment
Employees who forgo enrolling within their 30-day grace period, whom may have lost
medical coverage due to life changing events such as; marriage, divorce, childbirth,
adoption, etc., are eligible for special enrollment. Employees must notify Human
Resources within 31 days from when the life changing event has occurred, otherwise it
will differ to Open Enrollment. For more specifics, refer to your Summary Plan
description.
Continuation of Health Coverage
Federal law generally requires employers with 20 or more employees to give employees,
spouses (including same-sex spouses) and dependent children the right to continue group
health benefits for limited periods of time under certain circumstances, such as voluntary
or some types of involuntary job loss, reduction in hours worked, death, divorce and
other life events. Employees ordinarily may continue their health coverage for up to 18
months when their employment is terminated.
Dental Insurance
Citizens’ High School offers a dental plan for eligible employees. Please refer to the
dental Summary Plan Description for an explanation of the plan benefits and limitations.
Short-Term Disability Plan (STD)
Short-term disability plan is provided for eligible full-time employees. Please refer to the
STD Summary Plan Description for an explanation of the plan benefits and limitations.
