               BEK Foods will not assume responsibility for any damages to personal property caused by the
               BEK equipment while in possession of the above-mentioned customer.  THE CUSTOMER
               MUST PROVIDE BEN E. KEITH COMPANY A CERTIFICATE OF INSURANCE
               SHOWING WORKER COMPENSATION AND GENERAL LIABILITY COVERAGE UP TO
               $1,000,000 AND ALSO NAMING BEN E. KEITH COMPANY AS AN ADDITIONAL
               INSURED.  The customer listed above will be responsible for any injuries that occur and for all
               damage to the equipment while the trailer is in their possession.

               Ben E. Keith Foods reserves the right to periodically check this equipment to insure that it is
               being used properly and reserves the right to remove any equipment in the event that it is not
               being operated in a safe or approved manner.

                DO NOT store any alcoholic beverages of any kind on Ben E. Keith Foods equipment.

               The following safety rules must be followed:
                   •  All doors must be latched when open.
                   •  The walk ramp must be used to enter and exit the trailer when possible.
                   •  Only BEK personnel can move the trailer.
                   •  Product loss will be the responsibility of the customer.
                   •  It will be the customer’s responsibility to keep the proper amount and grade of diesel fuel
                       and oil in the refrigeration unit as instructed by BEK transportation/fleet personnel unless
                       otherwise negotiated.
                   •  A $250.00 service fee will be charged for a certified mechanic to come to the site if the
                       unit runs out of fuel and requires the fuel system to be re-primed.  Priming and restarting
                       the refrigeration unit if the trailer runs out of fuel must be done by a certified
                       refrigeration mechanic.  Refrigeration Unit will run between 40-50 hours under normal
                       conditions with a full tank of diesel fuel.
