mental and/or intellectual disabilities. Our activities and programs include: outdoor recreation, individualized
support services, adaptive technology, environmental modifications, hourly respite, individualized residential
alternatives, day habilitation, community habilitation, and medicaid service coordination. Our offices are located
at: 4 Commerce Lane, Canton, 3 Commerce Lane, Canton, 3 Remington Avenue, Canton.

CHC of the North Country provides quality, affordable health care to all individuals regardless of ability to
pay. We serve St. Lawrence, Franklin and Jefferson counties.

A common misconception about our organization is that we only provide services to low income individuals with
Cerebral Palsy. While we are especially committed to providing services to low income and medically under-served
individuals, we work with people from all income levels. Additionally, our services expand far beyond individuals
with cerebral palsy to individuals with all forms of intellectual and/or development needs.
