ATFForm4473 Section A
1 Transferor’s Full Name
No initials, except Jr. or Sr. Must also include the suffix such as “Jr., Sr., II, III.”
Must enter the complete name. If legal name is an initial only, enter “IO” after initial.
Name must match the ID.
If the gun form is manually filled out and if the name is written illegibly, the coworker may print
the name next to it. (See Instructions: Section A, Question 1)
If no middle name then the customer must enter “NMN”.
The customer that places the firearm on Layaway is the only person that can make the paid
out and fill out the ATF 4473. (Firearms Transaction Procedures pg. 6)
2 Current Residence Address
Can use initials “Rd., Blvd., St.” (Same as US Postal Service)
The state can be abbreviated. (Same as US Postal Service)
The city and county cannot be abbreviated.
Address should match the ID. Supporting documentation may be used if the address does not
match. (See Supporting Documentation)
Address on the 4473 Gun Form must match the Point-of-Sale Gun Book.
NO PO BOX. A post office box by itself is not acceptable. Supporting documentation may be
used to satisfy the address requirement. (See Supporting Documentation)
Supporting Documentation:
Example of documents that may be accepted include a vehicle registration, a recreation
identification card, a fishing or hunting license, a voter identification card, or a tax bill. However the
document in question must be valid and must have been issued by a government agency.
(ATF rul. 2001-5)—Supporting documentation is entered in 20b.
If on active military duty the State of Residence also is the State in which the permanent duty
station is located.
If the Armed Forces member does not reside in his/her permanent duty station, in your state, the
buyer must enter both the permanent duty station address and residence address.

SALE: A coworker may not sell a handgun to an out-of-state resident. (Also applies to a buy-back.)
SALE: A coworker MAY sell a long gun (not a handgun) to a customer that resides out-of-state if
prior authorization has been obtained from the Firearms compliance officer and/or Legal.
PAWN: Out-of-state residents can pawn a firearm (long gun & handgun) with approval from
Firearms Compliance. (Firearms Transactions Procedures pg. 4)
PAWN: A handgun pawned by an out-of-state resident may only be redeemed by the original
Pledgor. Long guns pawned by out-of-state residents may be redeemed by holders of a pawn
ticket (If state law allows), if prior authorization has been obtained from the Firearms compliance
officer and/or Legal.
3 Place of Birth
The state can be abbreviated. (Same as US Postal Service)
The Foreign country is entered if not born in the USA.
If born in the USA then the box for Foreign Country may be left blank or answered “N/A.”
