4 Height

 Customer enters current height.

 Does not necessarily have to match the ID.

5 Weight

 Customer enters current weight.
 Does not necessarily have to match the ID.

6 Sex

 Customer enters their sex.
 Verify with the ID.

7 Birth Date

 Must match the ID and the Point-of-Sale Gun Book.
 18 yrs. old or older for Long guns. 21 yrs. old or older for handguns.
 Common error: customer enters the date of the transaction.
 Coworkers that are 18 yrs. old or older may show and sell a handgun or long gun.
 Note: Alabama, must be 19 yrs old to pawn. Must be 21 yrs old to redeem a handgun.

8 Social Security Number

 This information is optional for the customer.

 Ensure the customer includes all the digits of their social security number.

9 Unique Personal Identification Number (UPIN)

 NICS Provides purchasers approved to have information maintained about them in the FBI
NICS Voluntary File (VAF) with a Unique Personal Identification Number (UPIN), which the
buyer should record in this box.
(See pg. 39 for more information on the UPIN number.)
 This box may be left blank or answered “N/A.”

10a Ethnicity

 The customer checks the appropriate box for ethnicity.

10b Race 10a and 10b must

both be answered.
 The customer checks the appropriate box for race.
 Can only check the available boxes. Can check more than one box.
