14 Transferee’s certification signature & date
The customer signs and dates here to certify that the information is correct.
15 If the customer takes the firearm on the date listed in box 15 then it must match box 37 of
pg. 3.
If the customer takes the firearm on a different day then he/she must sign box 22 and date
box 23.
16 Type of Firearm to be transferred
Check the appropriate box that describes the type of firearm(s) to be transferred.
If transferring both a handgun and long gun mark both the handgun and long gun. (Check
all that apply)
A pistol grip shotgun is checked as “Other.” (See pg. 36 for rules on pistol grip shotguns)
17 Location of sale if at a gun show
This rarely applies to First Cash. If this does not apply you may leave this blank or enter
“N/A.”
18a Identification
A Social Security card cannot be used.
A passport by itself is not acceptable, supporting documentation may be used to
satisfy the ID requirement.
Supporting Documentation:
Example of documents that may be accepted include a vehicle registration, a recreation
identification card, a fishing or hunting license, a voter identification card, or a tax bill.
However the document in question must be valid and must have been issued by a
government agency. (ATF rul. 2001-5)—Supplemental documentation is entered in 20b.
The customer must provide a valid gov’t-issued photo ID that contains the buyer’s
name, residence address, & date of birth.
Enter the expiration date, if any. If none you may leave blank or enter “N/A.”
The type of ID (ex. Driver’s License or DL, Identification or ID) must be entered and
the state it is from (can use abbreviations for the state.)
Verify that the picture on the ID matches the customer.
Common error: the birth date is sometimes written as the expiration date.
If the concealed handgun license or carry permit qualifies as the ID it must also be
included in question 23.
