        11          Questions: Certification of Transferee



                 DO NOT COACH THE CUSTOMER ON HOW TO ANSWER THE QUESTIONS.
                 For the customer redeeming out of pawn they should answer question 11.a   “Yes.”  (See Instructions:
               Question 11a.)

              Question 11.a needs to be a “Yes.”  If answered “NO” you cannot release the firearm. (Exception for

               repairs. See Instructions: Question 11a.)

              Question 11.b through 11.i needs to be a “NO,” if any are answered “Yes” you cannot release
                 the firearm. (Definitions for the questions can be found in the Instructions on the gun form.)

       12a          Citizenship



            If a US Citizen, then the customer should mark the “United States of America” box.
                If not a US Citizen, then the customer should mark “Other Country/Countries” box.

            If not a US Citizen, the customer still needs a valid DL or ID along with a US-issued alien
                 number or admission number that is to be entered on question #13.

       12b 12c         Questions: Certification of Transferee

       12d1 12d2         Non-immigrant Aliens                            Note:    A Non-Immigrant alien is a rare


                                                                                     occurrence in our shops.

            If question 12.d.1 is answered “NO” then question 12.d.2 is answered “N/A” and customer

              proceeds to question 13.

            If the question 12.d.l is answered “Yes” then customer must answer “Yes or No” to question 12.d

            If the customer answers “Yes” to 12.d.2 then must provide documentation that establishes legal

              status in the USA. (visa, work status, etc.)  Customer enters documentation in question 13.

            Non-immigrant aliens are persons traveling temporarily in the United States for business, or

              pleasure, persons studying in the United States who maintain a residence abroad, and certain

              foreign workers.  The definition does NOT include permanent resident aliens. (Instructions: Question 12.d.1)

            Must also provide a valid hunting license or permit or is an official representative of a foreign

              government, or foreign law enforcement officer of a friendly government entering the US on
              official law enforcement business. (This information is entered in 18c.)


       13          US Issued Alien Number or Admission Number


               If a non-citizen of the US the customer must provide a US-issued alien number or admission
                  number.

               The customer must enter this number on this line.

               If a non-US resident does not provide one of these numbers, do not complete the transaction.
                  (There is no need for a background check if the transaction is stopped)

               A coworker has no responsibility for verifying that the Alien number or admission number
                  provided is genuine. (FFL newsletter Dec 2002 Issue I)

               This box may be left blank if a US citizen or answered “N/A.”                               8
