       18b       Alternate Documentation
                  If the driver’s license or other valid identification does not show the current address it may
                 be supplemented by another valid, government-issued document showing the transferee’s
                 residence address.
                  Example of documents that may be accepted include a vehicle registration, a recreation
                 identification card, a fishing or hunting license, a voter identification card, or a tax bill.
                  However the document in question must be valid and must have been issued by a
                  government agency. (ATF rul. 2001-5)
                If no alternate documentation is needed then this question may be left blank or answered

                  “N/A.”



                                                             Note:     A Non-Immigrant alien is a rare

        18c      Non-Immigrant Aliens                                     occurrence in our shops.

                A non-immigrant alien must still provide documentation that establishes legal status in the

                  USA. (visa, work status, etc.)  Customer enters documentation in question 13.
                If the customer is a US citizen this question may be left blank or answered “N/A.”

                Non-immigrant aliens are persons traveling temporarily in the United States for business, or
                  pleasure, persons studying in the United States who maintain a residence abroad, and

                  certain foreign workers.  The definition does NOT include permanent resident aliens.

                Must also provide a valid hunting license or permit or is an official representative of a
                  foreign government, or foreign law enforcement officer of a friendly government entering
                 the US on official law enforcement business.  This documentation is entered here and a
                 copy of the documentation is attached to the 4473 Gun Form.


        19a        NICS or State Agency primary contact date


                Enter the date NICS or State Agency was first contacted for the background check.

                A NICS check is valid for 30 calendar days for the intended transaction.  After 30 days a new
                  NICS check must be conducted.






        19b       NICS or State Agency Transaction number

                   Enter the Transaction number provided by NICS or state agency.
                   Some state agencies may not provide a number.
                   If your state agency, after a delay, gives a different approval transaction number enter it in
                 21b next to the original entry.
                   Once NICS or state agency transaction number has been obtained, we are required to
                 keep the form in file even if the firearm is not transferred.
