OUR NETWORK COMMUNITY HEALTH CONNECTIONS Action Health Services – Provides substance abuse counseling and personal addiction identification, while working through the stages of addiction and rehabilitation in order to prevent the outcome of relapse. This program works with other local agencies who have the availability to provide long-term services to recently rehabilitated individuals. FITCHBURG PUBLIC SCHOOLS AFTER SCHOOL LEARNING CENTERS 21st Century Scholars After School Program – Helps students meet state and local standards in subjects such as reading and math. Increases student achievement in literacy, math, communication, and problem-solving. Assists with homework and the development of effective habits for learning. Fitchburg High School Plato Credit Recovery Program – This program’s unique approach addresses the needs of students who had been or are on the verge of dropping out of high school. Under the guidance of a licensed math teacher, ELA teacher and a guidance counselor, students complete classes they need to graduate. GARDNER COMMUNITY ACTION COMMITTEE Emergency Assistance – Serves low-income clients, addressing issues of poverty such as hunger/ malnutrition, along with immediate crisis such as rental eviction, house foreclosure, and utility terminations. Provides for basic human needs. Fellowship Table Program – Provides a free, nutritious, balanced meal for individuals in need of a safe environment. Predominately addressing hunger and malnutrition, this meal site also serves as an outlet for isolated individuals to share a common meal, socializing with others in a hospitable location. Medical Transportation – Coordinates travel arrangements to and from out-of-town medical appointments for elderly and disabled individuals in rural areas. GIRL SCOUTS OF CENTRAL AND WESTERN MA Girl Scout Leadership Experience – Provides girls with engaging, challenging, and fun activities that help them grow as people and as leaders, in addition to learning the value of serving others. HEART OF NEW ENGLAND COUNCIL, INC. (BOY SCOUTS OF AMERICA) Boy Scout Leadership Experience – Scouting is a family-based, year-round program positively affecting every area of a young person’s life. Scouting provides challenge, adventure, and lessons that last a lifetime and help young people fulfill their potential, in addition to learning the value of serving others. HOUSE OF PEACE & EDUCATION, INC. HOPE for Kids – An after school program for low- income children in Grades K-8 in Gardner and neighboring communities that is committed to a “whole child” approach and works closely with families to bridge the gap between home and school. HOPE Summer Camp – Provides a positive summer camp experience for those who would not otherwise have one. Provides breakfast and lunch to children who might not get enough food during the summer. LITERACY VOLUNTEERS OF ORANGE-ATHOL Literacy Volunteers – Promotes literacy skills that empower adults to realize their educational, vocational, and personal goals. Provides free, confidential, and individualized tutoring in basic literacy and English to Speakers of Other Languages (ESOL) to adults in North Quabbin communities through the use of professionally trained and supported volunteer tutors. LOAVES & FISHES FOOD PANTRY Food Pantry – Provides groceries and referrals to people in need.
