How to find a provider
1. Go to empireblue.com/find-doctor
2. You can look for a doctor by using either:
• Search as a Member: Log in with a username and password or with the member number on your ID card 1
• Search as a Guest:
• Select the type of care you are searching for - Medical
• Select the state you want to search in – Your home State
• Select the type of plan you want to search with - Medical (Employer-Sponsored)
• Select the plan/network
• Select “Blue Access” for the Bronze or Low plans if you reside in any of the New York counties listed
below.
• Select “National PPO (BlueCard PPO)” for the High plan and for all other plans if you do not live in any of
the New York counties listed below.
• For the Middle plan, you have the option of utilizing the “Blue Access” or “National PPO (BlueCard PPO)”
network. If you reside in Suffolk County, New York the “Blue Access” does not cover Stony Brook hospital
and affiliated medical offices. All other members across the country and in New York that do not need
access to Stony Brook hospital and affiliated medical offices should enroll in the “Blue Access” Network.
New York State Counties that are a part of the Blue Access Network: Albany, Bronx, Clinton Columbia, Delaware, Dutchess, Essex,
Fulton, Greene, Kings, Montgomery, Nassau, New York, Orange, Putnam, Queens, Rensselaer, Richmond, Rockland, Saratoga,
Schenectady, Schoharie, Suffolk, Sullivan, Ulster, Warren, Washington & Westchester.
In order to obtain accurate provider listing results, it is best to search as a guest on empireblue.com/find-doctor if you are changing plans
or networks.

Healthcare is now at your fingertips! Access board-certified doctors 24 hours a day, 365 days a year, by web, phone or mobile app. This
service can be used when you’re experiencing a wide range of conditions, including the following:
• Cold & flu
• Bronchitis
• Allergies
• Pink eye
• Skin rash
• Acid Reflux
• Sinusitis
Cost of visits vary depending on medical plan. There is no cost in the EPO & PPO plans.
HDHP employees will have a $59 copay.
