Dental – MetLife
Good health includes good dental health. That’s why Baseball offers the MetLife Preferred Dentist Program (PDP).
Under this program, like your PPO plan, you may visit dentists in the MetLife network or you may seek care outside
the network. You make the choice each time you need dental services.
Although your deductibles and copays are the same no matter whom you visit, services performed by network
providers typically are 10% to 35% less expensive than the average dentist charges in your area. Therefore, you
should save on out-of-pocket costs by using professionals in the MetLife network of dentists. (Currently, about
135,000 dentists from across the country participate in the MetLife network.) To locate a participating dentist, call 1-
800-942-0854 or visit www.metlife.com and click on My Benefits Dental Center.
REMEMBER!
Orthodontia plan benefits are paid as follows: up to 20% of the total
benefit will be paid for the initial placement of the appliance. The
remaining plan benefits will be paid in equal installments each
quarter, over the remaining course of treatment.
You and your dentist may request a pre-determination estimate of
benefits so you will know just what your dental plan will pay before
you start treatment. As part of the process, MetLife may suggest
alternatives for your care. Pre- determination of benefits is
recommended for procedures that are expected to cost $300 or
more.
