CORE BENEFITS







        Core Benefits are provided to eligible  team members if you work full-time (regularly working an
        average of 30 hours or more per week). Average hours are evaluated on a quarterly basis. You are
        automatically enrolled in these benefits if eligible.

          Basic Life and AD&D Insurance

        Life insurance is an important part of your financial
        wellbeing,  especially  if  others  depend  on  you  for              Don’t forget to name a

                   Disability Insurance


        Disability  insurance  provides  income  replacement
        should  you  become  disabled  and  unable  to  work
        due  to  a  non-work-related  illness  or  injury.
        Castlerock  provides  eligible  associates  with
        disability  coverage  at  no  cost  as  shown  below.
        Coverage  is  automatic;  you  do  not  need
        to enroll.


           Coverage                  Benefit

                       60% of your weekly salary, to a
                        maximum of $1,500 per week
          Short-Term    If approved, benefit payments begin on

                       Benefits continue for up to 11 weeks
                       60% of your base salary, to a maximum
                        of $6,000 per month if you are disabled
                        and are unable to work for more than 90

                        Workers’ Compensation.
